Chief Population Health Officer, Katherine Suberlak, LCSW, was appointed in 2015. However, she joined PCC as an AmeriCorps member in 2004 and returned to the organization as a graduate of the University of Chicago’s School of Social Service Administration. Ms. Suberlak established PCC’s behavioral health programming in 2007 and ushered in its substantial growth over ten years. Now, she maintains oversight of this integrated program, as well as our Illinois Breast and Cervical Cancer Program, Community Outreach Health Education Program, and Care Management Program. As PCC's first Chief Population Health Officer, Ms. Suberlak is responsible for maintaining and growing these services to help PCC achieve a Culture of Health within our target communities.
