PCC's Hepatitis C Program offers complex and integrative treatment of viral hepatitis. The program has a multidisciplinary team including physicians, nurse practitioners, specialty pharmacists and clinical nurses with expert knowledge in the treatment and management of Hepatitis C.
The program is the first of its kind to offer comprehensive services using a collaborative and personalized approach to treatment of viral hepatitis C. We integrate behavioral health, primary care and substance use treatment when needed. PCC's dedicated team can screen, treat, and manage patients with Hepatitis C all within our clinic using national treatment guidelines without the need for referrals. Our program is designed with you in mind. Our goal is to reduce treatment barriers by using our partners to provide expert, comprehensive care throughout the treatment process, making sure you feel supported every step of the way.

- Pre- and Post-Treatment Care
Patients will be referred to the Hepatitis C Program by their primary care provider. The patient is then added to our treatment list and contacted by program staff. The patient will undergo an intake process that includes labs, imaging, counseling, and education. During intake, follow-up appointments are planned. Once the patient is approved for the program, they are closely monitored by our program staff. Our program is designed to be comprehensive, patient-centered care with an extensive team that includes: HCV Program Coordinator, HCV RN, Specialty PharmD, HCV provider, behavioral health support, and case management support by ECHO-Chicago.
