Chicago, IL – As a complement to providing primary health care for residents of the Austin community, PCC Community Wellness Center (PCC) is celebrating National Nutrition Month® at its Austin-based health center. On March 13, March 20, March 27, and March 28, dietetic interns from Dominican University will coordinate activities to provide patients and staff with practical tips for eating healthier. Participants will have the opportunity to test their nutrition knowledge, taste some nutritious snacks, play games, win prizes, and have pressing questions answered by dietitians.
Chicago, IL – The PCC Austin Farm at PCC Community Wellness Center (PCC) is launching its third season of urban gardening workshops to encourage residents of the Austin community to grow their own fruits and vegetables for a healthier lifestyle. Held conveniently during lunchtime, the Lunch & Learn workshops teach individuals how to grow local, sustainable produce using organic practices. Sessions are free and open to the community. The first session of the season will be held on Thursday, March 23.

Berwyn, IL - March 18, 2015 – On Friday, PCC Community Wellness Center (PCC) held a grand opening for the Birth Center at PCC , Illinois’ first free-standing birth center.At the grand opening, U.S. Sen. Chicago, IL – PCC Community Wellness Center’s (PCC) community farm, the PCC Austin Farm invites 14 to 18 year-old students who attend high school or live in Chicago’s Austin neighborhood to participate in a contest to redesign the farm’s reusable tote bag. Chicago, IL – May 1, 2017 – PCC Community Wellness Center (PCC) was pleased to receive $50,000 in renewal funding from Searle Funds at The Chicago Community Trust. Funds will support general operations of PCC’s Care Management Program expansion.
Melrose Park, IL – A major cornerstone of PCC Community Wellness Center’s (PCC) services is providing comprehensive maternal and child health services. As a result of this commitment, PCC Melrose Park Family Health Center was recognized by the Chicago Area Breastfeeding Coalition (CABC), which presented Dr. Tamajah Gibson and her provider team with the 2016 Breastfeeding-Friendly Physician Award for their outstanding support of breastfeeding mothers and babies.
Chicago, IL – The PCC Austin farm will be hosting Women and Herbs, a free community workshop on Thursday, May 18, 2017 from 2 p.m. to 4 p.m. During the workshop, participants will have the opportunity to plant their own seedlings, learn about how to use herbs for health and wellness, and tour the farm.
Chicago, IL - PCC Salud Family Health Center, part of the PCC Community Wellness Center network of non-profit community health centers, celebrated the grand re-opening of its newly renovated facility on May 13, 2013 with an open house. Luis Arroyo (D), Member of the Illinois House of Representatives, 3rd District, attended and addressed the crowd who assembled to tour the facility. These fresh vegetables are harvested locally, from the PCC Austin Farm. Beginning June 6, visitors can choose among a variety of seasonal items, including lettuce, scallions, garlic, kale, and mustard and collard greens.

During this season’s kickoff on June 7, PCC Community Wellness Center (PCC) unveiled its new farm logo, designed by William Barker from Austin Multiplex High School. As the first place winner of PCC’s art contest, Barker was awarded a $700 scholarship. Two other students from the same school, Ebony White and Jacquan Johnson, second and third place winners respectively, were presented with $350 scholarships.

Oak Park, IL - In 2011, PCC Community Wellness Center (PCC) committed to providing a new model of care, patient-centered care, at its health centers. Following guidelines set by the National Committee for Quality Assurance (NCQA), five PCC health centers were recognized as Patient-Centered Medical Homes (PCMH) in August 2013. This fall, PCC Austin and Lake Street Family Health Centers further enhanced their services in order to obtain Level 3 recognition by NCQA.

Attendees will sing and dance along with local songstress, Melody DeRosa, also known for entertaining at the II Vicolo Trattoria in Oak Park.

Chicago, IL - National Breast Cancer Awareness Month is recognized annually throughout October, reminding PCC Community Wellness Center of their commitment to women’s health and the importance of the “pink” services they provide. Some of the services PCC provides to uninsured and underserved women in Westside Chicago and the near west suburbs are breast cancer education, instruction on self- breast exams, and routine clinical breast exams. PCC is a proud partner of the Illinois Breast and Cervical Cancer Program (IBCCP).
Oak Park, IL - The first newborn cries were heard in the Birth Center at PCC, Illinois’ first free-standing birth center, as two families welcomed their babies on the rainy fall morning of October 18. Sophia Azalia, daughter of Delilah Noyola and Ivan Ramos was the first baby born at the birth center, located at PCC South Family Health Center in Berwyn. Less than one hour later, Natalia and Roberto welcomed their son, Ryland.
Chicago, IL - Yesterday PCC Community Wellness Center (PCC) held a groundbreaking ceremony for its latest project to improve health outcomes—the PCC Austin Community Farm. Located in Chicago’s Austin neighborhood in an 8,000-square-foot lot across the street from PCC Austin Family Health Center, the farm was made possible by a $350,000 grant from the Humana Foundation. PCC is partnering with the Chicago Botanic Garden, which will provide participants from its urban agricultural program, Windy City Harvest, to install and maintain the farm.
Chicago, IL – PCC Community Wellness Center joins Norwegian American Hospital and other local agencies to welcome evacuees from Puerto Rico needing assistance finding community and health care services. They are welcoming evacuees in the Humboldt Park Field House’s Hurricane Relief Center. PCC has a welcome table set up, ready to provide evacuees with referrals for primary care, oral health care, behavioral health care, and healthy housing. Staff from PCC West Town Family Health Center will be in the relief center on Mondays and Tuesdays from 10 a.m. to 4 p.m. until December 19.

11/02/16 Birth Center at PCC celebrates second birthday

Berwyn, IL – On October 27, the Birth Center at PCC celebrated its second year of operations with a festive birthday party for birth center patients. Guests were presented with a Birth Center at PCC t-shirt for their babies, had the opportunity to take a family portrait, and enjoyed refreshments. And of course, there was birthday cake!
Chicago, IL - Earlier this year, four Chicago hospitals, six family health centers and one behavioral health provider came together to create MyCare Chicago, an Accountable Care Entity (ACE) and new managed care healthcare plan for individuals and families based in Cook County. MyCare is for children, parents and caregivers who are Medicaid eligible or on Medicaid or All Kids and who currently have an HFS medical card.
