Dr. Swartz is a family medicine physician who joined the PCC health care team in 2008. She earned her medical degree from the University of Vermont College in 2005 and completed residency at the Medicine Central Maine Medical Center. Dr. Swartz serves as PCC’s Medical Director of Electronic Health Records.
Dr. Smrha is a family medicine physician who received his medical degree from Chicago College of Osteopathic Medicine at Midwestern University. Dr. Smrha completed residency at West Suburban Medical Center Family Residency Program.
Dr. Sivasubramaniam completed her residency at PCC as part of the West Suburban Family Medicine Program. After graduating from Northwestern University, she earned her medical degree from Loyola University Chicago – Stritch School of Medicine.
Dr. Lizbeth Rodriguez is a Family Medicine Physician, Fellow of the American Academy of Family Physicians and Medical Director of PCC Salud. She received her medical degree from the University of Illinois at Chicago (UIC-COM) and completed her residency at West Suburban Medical Center/PCC Salud Family Health Center in 2017. She has a Bachelor's of Science degree in Clinical Laboratory Science with minor in psychology and biochemistry from DePaul University. Her professional interests include full-spectrum family medicine, underserved and women's health, procedures, and teaching. Dr. Rodriguez is fluent in Spanish and her hobbies include jigsaw puzzles, hikes and traveling.
Dr. Reniva earned his medical degree from the University of Santo Tomas in Manila, Philippines in 1991. He completed an internship at Lutheran General Hospital in Park Ridge, IL and residency in family medicine at Jackson Park Hospital Medical Center in Chicago.
Jerome Pietrusiak is a Licensed Clinical Social Worker who earned his Master of Social Work from Loyola University of Chicago in 1983. He completed clinical rotations at Riveredge Hospital. Jerome teaches social work courses at Concordia University Chicago and participates
Annette Payot provides leadership to PCC's Midwives as the Director of Midwifery. She is a certified nurse-midwife and Women's Health Nurse Practitioner who joined PCC in 2009. Annette completed her nursing and midwifery education at the University of Pennsylvania and the University of Illinois at Chicago in 2007.
Specializing in pediatric care, Dr. Parker-Renfroe joined the PCC health care team in 2004. She earned her medical degree from the University of Illinois College of Medicine at Chicago in 1992. Dr. Parker-Renfroe completed residency in pediatrics at Rush University.
