Chicago, IL – PCC Community Wellness Center joins Norwegian American Hospital and other local agencies to welcome evacuees from Puerto Rico needing assistance finding community and health care services. They are welcoming evacuees in the Humboldt Park Field House’s Hurricane Relief Center. PCC has a welcome table set up, ready to provide evacuees with referrals for primary care, oral health care, behavioral health care, and healthy housing. Staff from PCC West Town Family Health Center will be in the relief center on Mondays and Tuesdays from 10 a.m. to 4 p.m. until December 19.

Chicago communities like Humboldt Park, Logan Square, and West Town are expected to receive an influx of evacuees in the wake of Hurricane Maria. PCC West Town Family Health Center provides primary care and behavioral health services to these individuals in these communities. “We have around 40 evacuees signed up for care at PCC, most who are older adults,” said PCC Office Manager, Yaneth Antunez. “Some people are coming in for behavioral health visits. We’re also assisting those who need help obtaining medication refills.” Once care is established, PCC can assist evacuees with medication refills at select Walgreens.

PCC West Town Family Health Center provides all services in Spanish and has financial counseling available. The Hurricane Relief Center is open every Monday and Tuesday from 10 a.m. to 4 p.m. at the Humboldt Park Fieldhouse located at 1400 North Sacramento Ave. in Chicago.
