Dr. Mayela Leal Chanchi graduated medical school with honors from the University of Zuilia in 2014. She completed a Pediatric Residency at Georgetown University in 2021.
Lisa Kozinski is a Family Medicine Physician. She received her medical degree from Rush Medical College in 2002 and completed residency at MacNeal Hospital in Berwyn, IL. Dr. Kozinski completed her undergraduate degree in Biology at Northern Illinois University. She speaks fluent Polish.

Oak Park, IL – On October 8, the PCC Foundation hosted its Annual Fundraising Gala, “Some Enchanted Evening.” Held at the beautiful Oak Park Country Club, the evening began with a cocktail hour, during which time guests had the opportunity to win numerous treasures in an impressive raffle. The social hour was followed by dinner and live music. The salad course was fully harvested from the nearby PCC Austin Farm. The banquet hall was filled with the soothing sounds of local songstress, Melody DeRosa, also known for entertaining at the II Vicolo Trattoria in Oak Park.

Located in Chicago’s Steinmetz College Prep High School, the facility provides access to comprehensive primary care and behavioral health services to residents of the Belmont-Cragin, Dunning and Montclare communities as well as Steinmetz high school students.
Chicago, IL - On July 5, 2016, PCC Community Wellness Center (PCC) completed renovations of its first school-based community health center: PCC Community Wellness Center at Steinmetz. The newly renovated facility located inside Steinmetz College Prep High School provides access to comprehensive primary care and behavioral health services to Steinmetz students as well as community members of all ages. The health center primarily serves the Chicago communities of Belmont-Cragin, Dunning, and Montclare. Staff includes an attending physician, family nurse practitioners, licensed clinical social workers, health education specialist, care coordinator, financial counselor, registered nurse, medical assistant, patient care representative, as well as management staff.
Dr. Lizbeth Rodriguez is a Family Medicine Physician, Fellow of the American Academy of Family Physicians and Medical Director of PCC Salud. She received her medical degree from the University of Illinois at Chicago (UIC-COM) and completed her residency at West Suburban Medical Center/PCC Salud Family Health Center in 2017. She has a Bachelor's of Science degree in Clinical Laboratory Science with minor in psychology and biochemistry from DePaul University. Her professional interests include full-spectrum family medicine, underserved and women's health, procedures, and teaching. Dr. Rodriguez is fluent in Spanish and her hobbies include jigsaw puzzles, hikes and traveling.
Veronica Galvalisi is a Board-Certified Family Nurse Practitioner (FNP). As a recipient of a fellowship funded by the U.S. Health Resources and Services Administration (HRSA), she fulfilled all of her training at PCC and graduated from the University of Illinois at Chicago in 2015.
