All proceeds will benefit the PCC Foundation. Established in February 2016, the PCC Foundation is organized and will be operated at all times exclusively for the benefit of PCC Community Wellness Center. PCC Community Wellness Center is a community-based nonprofit health care organization that provides primary care, oral health, behavioral health, support services, and health education to medically underserved communities in West Side Chicago and the near west suburbs. In 2015, PCC Community Wellness Center served 47,184 individual patients. This number continues to grow as the demand for and breadth of our services increase.

As a vehicle for fundraising and community building, the PCC Foundation will extend the health center’s reach and capacity. With raised funds, the foundation will award grants to worthy PCC Community Wellness Center initiatives, such as health promotion, illness prevention, the provision of direct services, quality improvement, and enhanced access to care.

The need for high quality, affordable health care is immense; therefore, PCC Foundation is seeking additional support from the community to sustain and expand our critical cause. This year, we are celebrating PCC Community Wellness Center’s first school-based community health center, PCC Community Wellness Center at Steinmetz. While the clinic has been caring for patients since November 2015, renovations were completed in July 2016. PCC Foundation is grateful to the partners who enabled PCC Community Wellness Center to establish this beautifully remodeled clinic located inside Steinmetz College Prep High School at 3030 N. Mobile, Chicago, IL 60634.

PCC Foundation anticipates support for the gala with attendance of up to 250 physicians, health care providers, and prominent civic and business leaders, in addition to PCC Community Wellness Center staff and local supporters from Chicago and the Oak Park-River Forest area.
