Chicago, IL – PCC Community Wellness Center (PCC) was pleased to receive $50,000 in renewal funding from Searle Funds at The Chicago Community Trust. Funds will support general operations of PCC’s Care Management Program expansion.
PCC believes improved access to health care will result from strategic care management of the care cycle. PCC’s robust and evolving Care Management Program includes the collaborative process of conducting health assessments, care planning, facilitation, care coordination, ongoing evaluation, and advocacy. These interventions enable PCC to provide comprehensive health services such as primary care, dental care, and behavioral health services.
PCC’s Care Management team has grown to include over 28 care coordinators and clinical care managers across 10 of PCC’s 12 health centers. “We are grateful to the Searle Funds at The Chicago Community Trust for its generous, ongoing support,” said Katherine Suberlak, PCC’s Chief Population Health Officer. “Care management continues to be a necessary support service for our patients. However, it remains non-reimbursable, therefore external funding is imperative.” Searle Funds at The Chicago Community Trust has partnered with thousands of organizations to enhance the quality of life for individuals and communities across metropolitan Chicago.
